By completing this enrollment form, I acknowledge that enrollment is for the 2026–27 school year. I understand that a $50 nonrefundable registration fee per student is required to complete registration. If a student is not admitted, the fee will be refunded. Please submit one check payable to The Learning Center (TLC) by August 1. (Mail to: Whitney Patterson 5232 3rd Street Verona, PA 15147). Registration is not complete until the registration fee has been received.
I understand that payment for each optional class must be made directly to the teacher. Payments are due by August 21. Mailing addresses will be provided after class registrations have been confirmed. I confirm that the information provided is accurate and complete to the best of my knowledge.
